Download the official form and identify the requester

The Chinese Embassy in South Sudan publishes an official two-page Foreigner Physical Examination Form PDF. Use it as an official form source, while following the document and submission rules of your own requesting university, scholarship or competent authority.

Do not assume the embassy hosting the download is the office processing your application. A university scholarship request, a visa checklist and a post-arrival residence process can ask for evidence at different times and in different ways.

Before the appointment: prepare an administrative checklist

Read the requesting institution’s current notice for the form version, completion language, document age, provider requirements and supporting reports. If a validity period is stated, use that rule and calculate it against the required submission date rather than a guessed travel date.

Ask the institution about an unclear requirement before incurring a repeat appointment. Keep your passport identity details available so the provider can complete the record accurately. The healthcare provider should advise on examinations, clinical findings and any medical questions.

On a small screen, scroll the table sideways to compare all columns.

Check identity fields and completeness

Inspect your name, passport details and other personal fields against the identity document used for the application. Keep spellings and dates consistent with the admission record. Ask the issuing provider to correct an error rather than editing a signed medical record yourself.

The official form includes personal details, a photograph area, examination entries and certification fields. Follow the requesting checklist for photographs, stamps, signatures and accompanying reports. A blank mandatory field can lead to a document query even when the scan is clear.

Prepare the upload without losing information

Scan every required page so edges, dates, signatures and stamps remain readable. Combine or separate files only as the portal requests. Inspect the actual uploaded version after compression: making a file smaller should not make the evidence unreadable.

Use the correct document field and keep the submission receipt. If the portal has a file-size limit, reduce resolution carefully while preserving legibility. Never replace the form with a screenshot of an appointment booking or an incomplete report unless the authority explicitly permits provisional evidence.

Validity and arrival: do not assume automatic reuse

Some processes specify a limited age for examination evidence or request additional verification after arrival. Read your own application instructions and ask the university which originals to bring. This guide does not establish one validity period for every Chinese institution.

If an application is deferred, check whether the existing evidence remains acceptable. A form that satisfied an earlier scholarship upload may not satisfy a later administrative stage. Keep the requester’s written answer with the document file.

Privacy and a useful correction request

Submit health documents only through the official channel specified for your application. Do not post the completed form, passport number or reports in a public student group. AspireCompass does not collect or assess these documents.

If the receiver reports a problem, ask for the exact missing field, page or authentication requirement. Send that instruction to the qualified issuing provider where a correction is needed. Medical interpretation and treatment questions belong with a qualified professional, not a scholarship blog.
